The Connecticut Healthy Living Collective (CHLC) is comprised of state, regional and local agencies and organizations dedicated to healthy aging in communities throughout our state, and to providing leadership and visibility for the challenges and opportunities faced by Connecticut’s growing aging population. Their programs help you grow in self-confidence, master new skills and improve your overall health – and your life.
If you have an ongoing health condition such as heart disease, high blood pressure, arthritis or something different, Live Well with Chronic Conditions will help you manage your disease better, improving your life overall. If you have diabetes, Live Well with Diabetes will help you become more self-sufficient in managing your life, helping you feel more in control and healthier. Live Well with Chronic Pain can help you better manage your symptoms. Also, if you want to learn new ways to help improve your health, financial security and overall well-being, consider enrolling in an Aging Mastery Program near you. And, Tai Ji Quan: Moving For Better Balance will help improve balance, mobility, daily functioning, and prevent falls. Are you experiencing chronic pain brought on by arthritis, stroke, neuropathy or another condition? If so, this workshop provides you with practical tools to help you develop self-management skills, while giving you the confidence and motivation you need to better manage symptoms, and meet the challenges of day-to-day tasks of living with chronic pain.
Classes encourage active participation, where mutual support and success build confidence in your ability to manage your health and maintain active and fulfilling lives. The workshop will not conflict with existing programs or treatment and is designed to enhance regular treatment.
The Aging Mastery Program® (AMP) is a 10-week series of incentive-based health and wellness classes for people age 55 and over. AMP can help you build your own playbook for aging well. It is a fun, innovative, and person-centered education program that empowers participants to embrace their gift of longevity by spending more time each day doing things that are good for themselves and for others.
Are you an adult at risk for falling, or are you afraid of falling? If so, this Program, which is an evidence-based fall prevention program, is for you. Years of research and community evaluations are behind this therapeutic movement program aimed at improving postural stability, awareness and mindful control of body positioning, functional walking, movement symmetry and coordination, range of motion around the ankle and hip joints and lower-extremity muscle strength, to improve balance, mobility and daily functioning and prevent falls.
